If you are an active member, you can complete this form online as part of the pension application process by signing in to My Account.

Eligibility

If you terminated your employment within 30 days of your retirement date, you must complete and sign this form to be eligible to start receiving your pension.

If you terminated employment more than 30 days before your retirement date, you do not need to complete this form.
