- Print, sign and date it
- Attach any required documents to the form
- Submit the form to BC's Public Service Pension Plan
Eligibility
You can add your spouse and/or dependant(s) to your extended health care and/or dental coverage if you currently have coverage, and your spouse and/or dependant(s):
- Have medical coverage through a provincial health plan in the province where they live (if they're applying for extended health care coverage)
- Are joining the plan within 60 days of your pension effective date or have had continuous coverage in a different plan (if they are applying after the 60-day period has ended)
If you have not yet applied for your pension, you can add your spouse and/or dependant(s) to your extended health care and/or dental coverage as part of your pension application.

If you are applying for coverage for you, your spouse and/or eligible dependant(s) 60 days or more after your pension effective date, check the Green Shield Canada My Benefit Plan booklet to see if you meet the eligibility criteria.
