What to have ready

  • Your social insurance number
  • The plan member's social insurance number or Person ID (if known)
  • Name and address of a witness who will be present when you sign the form
    • Your witness must be of sound mind and at least 19 years old
  • Access to a printer

Submit the form

  1. Complete Form P2 Request for designation as limited member online
  2. Print the form
  3. Sign and date the form with a witness
    Your witness must also sign the form. Keep a copy for your records.
    Note: We cannot accept digital signatures
  4. Submit your signed form
