Submit a form to buy service
Use this application to apply to buy back your service from a leave or periods when you worked but didn’t contribute to the plan.
What you need to know
Sign in to My Account and use the Purchase cost estimator, if you’re eligible.
Review Understand the process for important details about leave types, deadlines and payment options.
Remember you must submit the application within the deadline to purchase service.
For lump sum payments, you must apply by whichever of the following deadlines comes first:
- Five years from the end of the leave period you're applying to buy
- 30 days after leaving the employer with whom the leave occurred
For continuous contributions during an Employment Standards Act (ESA) leave:
- Within 30 days of starting the leave
Submit the form
- At the bottom of this page, click Purchase of Service Application Package, and save to your computer.
- Complete Part A.
- Send the entire package to your current employer Your employer will complete Part B and send the form to the plan.
By submitting the application, you are not obligated to buy the service. Once we have processed your application, we will send you a statement outlining the cost and next steps, if you choose to proceed.
If you are registered for My Account, you will receive an email notification when it is available in Message Centre.
If you have chosen to make continuous contributions while on an approved leave of absence under the Employment Standards Act (ESA), you will receive regular statements approximately once a month.
Eligibility
Use this form if you are applying to buy back service for:
- A leave of absence, either as a lump-sum payment or through continuous contributions
- Past service
- Non-contributory service
- Arrears
- The type of service you want to buy
- The start and end dates of your gap in service
